How to Use the School Survey Template

• First, insert the school name, district, and date at the top of the page. Type a sentence or two stating the purpose of the school survey.
• The next step would be for the parents to fill out the date, grade, and name of the teacher handing out the survey.
• Below the header, the “areas of service” section is placed on the left side of the paper, and the quality rating system is on the right side of the page.
• Record your rating for Overall Impressions.
• Select your ratings for the second section of the survey, which involves school resources.
• Ratings involving Life Skills is the next section on the survey that you will cover.
• The final section regarding the student is the behavior section.

Tips for Using the School Survey Template

• The survey is currently is split into two pages and has multiple sections.
• You can change the sections to fit the needs of the school.
• When you get the surveys back from the parents, the ideas of the parents will become obvious. Use these comments and ratings to help make your school grow stronger.
• Parents are urged to leave comments in the “Optional Comments” section. Use these to further shape your educational system.
