At the top of the page, you can enter the information that is listed above. For example, you will start by entering the name of the patient, their contact information, their date of birth, etc. With this, you will always have a reliable reference sheet to refer back to in a pinch.
To print the document to enter the items by hand, simply scroll to the top of the page and select the “File” tab and then the “Print” option from the list that will appear.
Now, go to the list of tables at the center of the document that are highlighted in yellow. You will use the tables to indicate whether the patient is suffering from certain pains.
To use an example, let’s say that the patients experience chronic pain in the stomach. You would ask certain questions and enter that information in the first table. You would then proceed to the next tables, listing the risk factor, any other acute problems, and perhaps referrals to another specialist.
