Using the Pain Level Chart

Now, scroll down to the middle of the page to find a helpful graphic. To chart your pains levels with a coordinated system that most doctors use, you will simply find the face that matches how you feel to describe mild, moderate, and severe symptoms.

When you have finished assessing your own pain levels, you can fill in the details in the final comments section of the box.
